Short answer
Who pays my medical bills after an injury settlement?
Medical bills related to the injury are paid out of the settlement, not by the at-fault driver as treatment happens. At disbursement the attorney's fee and case costs come out first, then hospital liens, health-plan reimbursement claims and treatment provided on a lien, and what remains is the client's net. Negotiating those liens down is often the single largest lever on how much money actually reaches the injured person.
How treatment gets paid while the case is pending
Health insurance, Medi-Cal or Medicare, med-pay coverage on the auto policy, or a provider treating on a lien are the usual sources. Each creates a different reimbursement right later.
Med-pay is worth checking early: it pays regardless of fault and is often the fastest way to keep treatment moving.
The reimbursement claims that follow
Hospital liens, ERISA and other health-plan reimbursement rights, Medi-Cal and Medicare recovery claims, and provider liens all attach to the settlement. They must be identified before disbursement, not after.
Government program claims have their own resolution processes and cannot simply be ignored; failing to address them creates personal exposure for the client.
Why lien reduction matters more than the headline number
Two settlements of the same size can produce very different client checks. Reductions negotiated on liens flow entirely to the client, which is why lien work is treated as part of the case rather than paperwork at the end.

- When do I receive my money?
- After the release is signed, the settlement funds clear, and liens are resolved. Ask your attorney for a written disbursement statement showing every deduction.
